A+ Afterschool Program Forms
Registration can be completed online or you can download and complete the A+ Printable Forms if you prefer. (All forms and payments must be completed prior to attending any program)

- If you would like to enjoy the convenience of automatic billing, simply complete this form. Upon approval, we will automatically bill your credit card on the first school day that an A+ payment is due (see forms for dates and more details).
